Provides care for non-urgent conditions and a “medical home” for adults with chronic conditions like diabetes and high blood pressure. Services include Routine Health, Urgent Care, Substance Abuse services, Mental Health referrals. Appointment Required. No Walkins. Nurse Practitioners evaluate, diagnose and prescribe medications. Insurance NOT accepted, Office visits are $50. Additional fees for X-Ray and lab tests and prescriptions.
